The Army EEO Modernization Plan is designed to upgrade the tools and methods used to support the Army's EEO program. These tools are designed to enhance customer understanding of the various aspects of the EEO program and provide a more rapid production of information.

This form modifies block 5 of the existing form. Requestors need to add their servicing HR Region, CCPO-ID, and EEO office ID. The form will be sent to HQ DA EEO through command EEO channels. Make sure all required blocs (such as bloc 17) are completed.
